District Health Information

Need a medication permit or health care plan, immunization and/or concussion information?  Health information and Health Care Plans plus medication permit forms can be accessed from the Health page on the District 12 website.

Did your child get any new vaccines or immunizations?

Please email or send in a copy of your child’s updated immunization/vaccine records to be updated here at school.  You can view your child's immunization record in your parent portal at any time.

Requirements for Non-Medical Immunizations Exemptions

There will be two methods for submitting a non medical (personal/religious belief) exemption for immunizations. Families can choose to:

 

1.     Submit the downloadable Nonmedical Exemption Form WITH a signature from an immunizing provider who is a medical doctor, doctor of osteopathic medicine, advanced practice nurse or delegated physician’s assistant.

OR

2.     Complete the CDPHE Online Immunization Education Module and provide the certificate of completion. Both the form for Option 1 and the Education Module for Option 2 are available at the CDPHE Vaccine Exemptions website at https://cdphe.colorado.gov/vaccine-exemptions

 

The exemption process needs to be completed through the links above. Families should save and print the appropriate form and submit it to their school each school year.
