Attention 7th-12th Grade Athletes Boys and Girls
Sports Physicals Offered on: Saturday August 3rd
8:00am – 11:30am
Cheyenne Mtn H.S. Gym $20.00(Monies collected benefit CMHS Athletics)
(No appointment necessary- first come, first served; All forms may be downloaded at www.cmsd12.org under the athletics tab- documents & forms; if parents are not present at physical, please complete the medical release & disclaimer form also)
Any Student wishing to participate in a sport next year must have a
current physical BEFORE practices begin. Physical will include Height, Weight, Blood Pressure, Pulse, Vision Screen, Orthopedic Assessment, and Physician Consultation (Doctors are MD’s, DO’s, and Orthopedic Surgeons from local area medical clinics)
